I commit to and understand the following:

• I will attend and participate in all team meetings, gatherings, events, and support/fundraising efforts. Missing meetings without a real approved excuse would disqualify you for participating in the mission trip or training opportunity.

 

• For International participants and/or the trip leaders to be prepared for this trip. Lack of participation in the meeting might result in forfeiting your place on this trip without a refund of payments made.

 

• I will meet all deadlines and due dates for proper travel documentation, applications, and team project assignments.

 

• I understand I am personally responsible for meeting all financial requirements not covered by support raising, deposit, fundraising, and scholarships. I also understand that all fund raised above what is personally needed would be used to help other team members who might be short or for field ministry expenses or other BOLI' mission work. 

 

• I understand my financial deposit is NON-REFUNDABLE.

 

• I will submit to the authority and wisdom of the leadership of Breath of Life both USA team leader and local leadership.

 

• I understand all reasonable safety precautions will be taken at all times by Breath of Life International and its agents during events and activities. I authorize any medical treatment by an accredited hospital and/or physician deemed necessary for the subject of the release in case of an emergency. I understand the possibility of unforeseen hazards and know the inherent possibility of risk.

 

• I acknowledge that participation in the above trip involves risk to the Participant (and to Participant’s parents or guardians, if Participant is a minor), and may result in various types of injury including, but not limited to the following: sickness, bodily injury, death, emotional injury, personal injury, property damage and financial damage and hold BOLI, its board members and all people associated with the organization harmless. 

 

• In consideration for the opportunity to participate in the above trip, the Participant (or parent/guardian if Participant is a minor) acknowledges and accepts the risks of injury associated with participation in the trip.  

 

• I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my participating in this mission trip with Breath of Life International.  
• I authorize Breath of Life International to use any pictures/videos taken of me during the trips to promote its mission and vision.

The Participant (or parent/guardian) accepts personal financial responsibility for any sickness or injury sustained during the trip. Further, the Participant (or parent/guardian) promises to indemnify, defend, and hold harmless Breath of Life International and its agents, employees, volunteers, or any other representatives (collectively included hereinafter in the term BREATH OF LIFE INTERNATIONAL (BOLI)”) for any sickness or injury related directly or indirectly out of the above trip, whether such injury arises out of the negligence of BREATH OF LIFE INTERNATIONAL (BOLI)”) or otherwise. If a dispute over this agreement or any claim for damages arises, the Participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process in keeping with biblical principles. If the Participant (or parent/guardian) and BREATH OF LIFE INTERNATIONAL (BOLI)”) cannot agree upon such a process, the dispute will be submitted to a three-member American Arbitration Association arbitration panel for final resolution in Kentucky.
